QEA has partnered with the Forsyth County Department of Public Health to offer COVID-19 Vaccinations to anyone ages 12 years and older.

This event is free and open to the public.

​

The first dose will take place Wednesday, June 16th with a follow up dose on Wednesday, July 7th.

​

The Vaccine Administration Form must be completed and submitted the day of your appointment.

 

Copies of the form will also be available onsite the day of the event.

​

The event will be held in the High School Gymnasium located at 5151 Lansing Drive, Winston-Salem, NC 27105.
